Schedule: Monday - Friday first shift hours with possible evening hours.

 

SUMMARY

Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.

 

QUALIFICATIONS EDUCATION and / or EXPERIENCE, LICENSURE AND CERTIFICATION

Graduate of an approved practical nursing program. Licensed to practice as a Practical Nurse in the State of Michigan. Successful completion of competency based orientation program. CPR certification or acquisition within 90 days of employment.

 

Good interpersonal skills. Ability to communicate effectively on both an oral and written basis. Ability to maintain a high level of confidentiality. Ability to manage several tasks simultaneously. Ability to work with high volumes of workload. Ability to listen and communicate with patients / significant others. Ability to cope with stressful situations and maintain a calm and professional demeanor. Ability to use software applications applicable for work area.

 

PHYSICAL DEMANDS / WORK ENVIRONMENT

Sitting in a normal seated position for extended periods of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard. Ability to move about. The employee must frequently bend and lift and / or move up to 60 pounds.

 

The noise level in the work environment is usually moderate. Possible exposure to bloodborne pathogens, hazardous materials, and sharps.

 

ESSENTIAL FUNCTIONS include the following. Other duties may be assigned.

Collects and monitors assessment data and reports physiological, psychological, sociocultural and spiritual needs of patients and families in coordination with and as delineated by the RN / Mid-level Provider / Physician.

 

Provides direct patient care under the direction of the RN / Mid-level Provider / Physician, consistent with policies and procedures, and unit based routines.

 

Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN / Mid-level Provider / Physician.

 

Documents care provided via computer / written patient record in a manner consistent with guidelines.

 

Greets and communicates with patients and families in a manner consistent with caring and respect. Communicates and collaborates with other members of the health care team in order to ensure continuity of care and coordination of service.

 

Cleans patient's environment / work area / equipment and communicates any equipment malfunctions to appropriate department / unit personnel.

 

Documents patient chargeable items according to standards.

 

Participates in selected quality monitoring activities under the direction of the RN / Mid-level Provider / Physician.

 

Demonstrates a high standard of moral and ethical behavior. Recognizes appropriate and ethical behaviors and identifies resources for resolution as needed. Advocates for privacy, confidentiality, and security of patient, staff and organization data.

 

Delivers care in a manner that is culturally sensitive and preserves patient autonomy, dignity and rights.

 

Maintains knowledge of current trends and developments in the field by reading appropriate books, journals, and other literature, and attending related conferences, seminars, and other related activities.

 

Performs such individual assignments as management may direct. Establishes and maintains effective working relationships within the organization.

 

Completes required continuous training and education, including department specific requirements.

 

Ensures that patients, their families, visitors, customers, and employees are treated in accordance with Spectrum Health core values and service standards.
